New Patient Information

Scheduling an appointment:

In order to schedule a new patient appointment, patients must call 706-425-4044 on Tuesdays from 12-1pm. Appointments are scheduled every Tuesday based on availability. All new patient appointments are scheduled three weeks in advance. Patients are seen by appointment only and no walk-ins are accepted.

Eligibility:

In order to be eligible for services, patients must be completely uninsured, at or below 150% of the Federal poverty level, and a resident in one of the following counties: Clarke, Barrow, Jackson, Madison, Oconee or Oglethorpe.

Required Documentation:

• Picture Identification
• Proof of income, such as paystubs or a notarized letter
• Proof of residency, such as a letter with a recent date, name and address, or a lease agreement

Fees:

Services are provided free of charge, although every patient is given the opportunity to contribute to their care either through financial donations or volunteering.
